On case #019248352, we inquired about connecting the "Employee Affiliation" questionnaire to incentives, so that the donor can be asked conflict-of-interest questions when redeeming an incentive.

Since questionnaires are currently only available within Giving and is not available under Incentives - we would like to make an enhancement request to add questionnaire functionality to Incentives.
